PROVIDENCE — Anchor Medical shut down Monday after serving Rhode Island patients for 25 years.
"We are deeply grateful for the trust you have placed in us over the years, and we want to ensure that your transition to new care is as smooth as possible," the health care provider posted on its website.

"For Anchor patients interested in transferring care or needing medical records, we want to help," Arches Medical, which has 10 locations across the state, posted on its website.
"Arches Medical was selected for their established relationships with many Anchor clinicians, their use of the same Athena electronic health system, and their commitment to continuity of care," Anchor Medical said.
