42 Violations Found At 1 Place: Manassas Restaurant Inspections

Prince William County health inspectors found as many as 42 violations during recent restaurant inspections.

MANASSAS, VA — In the first days of 2025, some restaurants in the Manassas area recorded violations during recent health department inspections.

Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually. Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

Here are the most recent inspections from the Manassas and western Prince William County area in the latter part of January, according to the Prince William Health District. Of note, Pho Nucuoi Saigon had 42 violations, including shrimp not heated to a proper temperature and food handling without gloves.

Another result was the "presence of pests" at Kabob Zone, according to health inspectors. The type of pest was not specified, but the violation covers insects, rodents or other animals.

Subway: 10094 Dumfries Rd Manassas, VA 20110

Violations:

  • There was no verifiable training of employees reporting diagnoses, illnesses or living with person with diagnoses and illness directly associated with foodborne illness.
  • No written procedure for clean up vomit and diarrhea was available during the inspection.
  • A container of meatballs was temp at 113F in the steam table that had been in the steam table since 7am.

  • Several of the hand sinks within the facility missing hand soap.
  • In several bulk food containers there was single service items that were not clean to the sight or touch laying in the food.
  • In the 2 door migali freezer there was several containers of food that were uncovered.
  • In the walk in cooler and 2 door sandwich prep cooler there was gyro meat 56.00 F, cooked spinach 45.00 F, diced tomatoes 48.00 F, salad 49.00 F, butter chicken 45.00 F, lamb chops 45.00 F, heavy cream 45.00 F, whole milk 54.00 F, cheese 48.00 F, salad 56.00 F, tzatziki sauce 56.00 F, gyro meat 45.00 F, lettuce 45.00 F.
  • There was containers of medication over the prep table in the kitchen.
  • In the winco hot holding unit there was presence of pests.
  • The employees that were in the kitchen engaging in food prep were not wearing hair restraints.
  • The facility was reusing single service mayo containers and single service cups as scoops.
  • The walk in cooler and the 2 door sandwich prep was not properly holding food items cold at 41F or below.

  • Observed food employees changing tasks without washing their hands, using improper cooling methods for beef broth, cold holding food (beef broth) at inadequate temperatures, and handling food (bean sprouts, jalapenos, and lime) with bare hands.
  • Observed an open-mouth drinking cup on the shelf above the prep table. The person in charge stated that it was in use.
  • Observed a food employee washing their hands in the three-compartment sink.
  • Observed food employees improperly washing their hands by rinsing their hands for only a few seconds without a cleaning agent.

  • Observed a food employee handle ready-to-eat raw bean sprouts, jalapenos, and lime with bare hands.
  • Observed a hand sink being used to store empty wiping cloth containers.
  • Observed a bowl sitting in a container of brown rice in the True two-door reach-in cooler.
  • Observed raw, portioned, beef, fish, and chicken outside of their original containers being stored in the same container in the walk-in freezer.
  • Observed raw bell peppers stored next to portioned, raw chicken outside of its original container in the walk-in freezer.
  • Observed the person in charge using the same, used single-use gloves after washing hands.
  • Observed a food employee handle a cleaned and sanitized cup with dirty gloves.
  • Observed the slicer in the kitchen soiled with food residue.
  • Observed shrimp being improperly cooked in the microwave. The shrimp was placed in the microwave without being rotated or stirred midway during cooking, did not reach a temperature of 165°F (temperature was taken at 157°F), and was not allowed to stand covered for two minutes after cooking.
  • The person in charge stated that this establishment uses non-continuous cooking for beef without written procedures that are approved by the Prince William Health District.
  • Observed beef broth that was cooked and cooled the previous day (2/18/25), and taken out of the walk-in cooler at 10:00am on the day of the inspection (2/19/25), cold holding on an unlit burner stove at 49°F at 10:47am.
  • Observed cooked beef brisket and brown rice cold holding in the True three-compartment reach-in cooler without date labels.
  • Observed a food employee's medicine being stored on the shelf above a prep table in the kitchen.
  • Observed a large pot of beef broth improperly cooling in the kitchen (temperature measurement: 100°F, made at 10:00am, temperature taken at 10:58am). The beef broth was left out to cool on an unlit burner stove.
  • Observed spices stored in a non-food grade container (a single-use bucket) in the kitchen.
  • Observed beef broth being stored in the floor in the walk-in cooler.
  • Observed food employees in the kitchen handling food without hair restraints.
  • Observed sponges at the three-compartment sink.
  • Observed wet wiping cloths being stored on prep tables.
  • Observed containers across from the three-compartment sink stacked on other containers while still wet.
  • Observed large pots being stored on the floor across from the three-compartment sink.
  • Observed reused single-service deli containers that were cleaned and sanitized stored across from the three-compartment sink.
  • Observed single-service MSG buckets used to store spices in the kitchen.
  • Observed a wooden shelf in the kitchen.
  • Observed the three-compartment sink sealed to the wall with tape.
  • Observed the slicer in the kitchen in a state of disrepair and rusty.
  • Observed the food dolly in a state of disrepair and covered in rust.
  • Observed cardboard in the kitchen underneath the slicer and below the hand sink.
  • Observed carbon and grime buildup on pans in the kitchen.
  • Observed the three-compartment sink leaking.
  • Observed an inadequate air gap between the water supply inlet of the three-compartment sink and the flood level rim.
  • Observed the floor in a state of disrepair with broken tiles and unfinished areas.
  • Observed a toolbox being stored in the kitchen.
  • Observed light bulbs that were not lit at the stove and burner areas.
  • Observed grime buildup on the walls of the kitchen.
  • The light intensity at the stove was measured at 22 foot candles and the light intensity at the smaller burners was measured at 30 foot candles.
  • Observed unshielded light bulbs in the kitchen above the stoves.

Chili's Grill & Bar: 10600 Sudley Manor Dr Manassas, VA 20109

Violations: No violations found. During today's follow up inspection, many of the violations noted at the routine inspection were corrected. Observed multiple ceiling tiles and wall areas of food prep area in disrepair and some ceiling tiles are still heavily stained. According to the Person in Charge, the ceiling tiles and walls will be replaced sometime in March 2025.
