18 Violations At 1 Place: Woodbridge Restaurant Inspections

Prince William County health inspectors found 18 violations at one location, including "visible soil" on walls and floors.

WOODBRIDGE, VA — In recent weeks, health inspectors found violations at some restaurants during inspections in the Woodbridge area.

Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually. Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
  • The handwashing sink located adjacent to the 3 compartment sink is blocked with a chair holding covered containers of soaking rice, preventing access by employees for handwashing.
  • Fries and other foods were uncovered in the Hoshizaki prep reach-in refrigerator. Various frozen foods were uncovered in the GE Turbo 1 door white upright freezer #1.
  • Tongs used at grill were observed to be used for both raw kabobs and cooked ready to eat kabobs.
  • Vaseline and vitamins for employee use were observed to be stored up front adjacent to single service items and foods.
  • Chutney sauce (made from yogurt base) made and placed in condiment containers observed to be cooling on ice at 51 degrees F. The condiment containers were stacked 3 levels high on top of the ice.
  • There were no temperature measuring devices (thermometers) located in the following cold or hot holding equipment: All 4 upright freezers; the thermometer in the Hoshizaki prep reach-in refrigerator is in disrepair.
  • The outer opening solid door in the back of the food establishment is not protected against the entry of insects and rodents (vermin) as daylight is visible at the bottom of the door.
  • Upon arrival, staff were observed to be using the prep table in the rear part of the dining room/hallway to prepare the ground beef Seekh kabobs. The freezers with locks previously allowed to be stored in the rear dining room area were found to be unlocked upon arrival. Another smaller freezer was added.
  • The yellow wet wiping cloths on the prep surfaces across from the cook line were not properly stored in the sanitizer bucket in between use.
  • The stand mixer and prep table are located in the rear dining room/hallway exposed to potential contamination from customers. Clean knives were stored down in between the prep table and Hoshizaki prep refrigerator, a location not capable of being properly cleaned/maintained.
  • Paper plate was being reused to scoop fries. Condiment containers were being reused as scoops in spice containers. Black single service bowl was being used to scoop dry rice out of bulk storage bin. Spice containers were being reused to store other foods such as sugar.
  • Foil is being used throughout the facility to cover bricks used at the grill; to cover the lower shelving of the prep table; to cover some other material and used as lids for the Rovsun table top steam table. Duct tape is being used on the handle to the walk-in cooler.
  • Wooden paddles were cleaned and stored above the 3 compartment sink ready for use with preparing foods.
  • The 3 plugs/stoppers for the 3 compartment sink are missing.
  • There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (quaternary ammonium).
  • The interiors of the freezers are unclean with an accumulation of food residue and ice build up.
  • The walls and floors at the 3 compartment sink and handwashing sink are unclean with visible soil.
