- Milk crates are being used to hold food throughout the kitchen and in the walk-in cooler.
- The lid to the white storage bin labeled "Azucar" in the dry storage area was broken and long longer easily cleanable.
- Cardboard, foil, paper, plastic and a thin veneer of wood with tape is being used to line shelving and other surfaces throughout the food establishment.
- The walk-in cooler was found in disrepair and not holding an ambient air temperature of 41 degrees F or below. The door to the walk-in cooler is rusted through leaving holes in the interior surface of the door. The door itself does not close properly.
- The 3 plugs/stoppers for the 3 compartment sink are missing.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (chlorine) used in the Ecolab warewashing machine.
