Expired Food, Contaminated Cooler: Woodbridge Restaurant Inspections
Prince William County health inspectors found all foods in a cooler affected by a mold or mildew contaminate and other violations.
WOODBRIDGE, VA — In recent weeks, health inspectors found violations at some restaurants during inspections in the Woodbridge area.
Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually. Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

Here are the most recent inspections from the Woodbridge and eastern Prince William County area, according to the Prince William Health District. Rayhan Grill was cited for food products in the cooler being covered by a mold/mildew contaminate, while Alamo Drafthouse Cinema was cited for expired foods as its only violation.
There were also violations that appeared at multiple restaurants, such as improper hair restraints for some employees as well as uncovered foods in coolers, inadequate sanitizer to properly sanitize dishes and more. Health inspections made suggestions for corrections in the reports.

Dok Khao Thai Eatery: 15200 Potomac Town Place, Suite 110 Woodbridge, VA 22191

Violations:
- The food facility has not established procedures for cleaning up vomiting and diarrheal events for employees to follow when these events may occur in the food establishment.
- Observed open container of cooked food for employee's lunch (according to the person in charge) stored on the top of the prep table in the kitchen.
- The handwashing sink on the right behind the kitchen door was used as storage for kitchen utensils.
- Observed raw chicken stored next to cooked chicken and cooked ready to eat beef on the same shelf in the walk in cooler and on the bottom shelf in the prep cooler located across from the cooking grill.
- The following foods were measured at improper cold holding temperatures: Cooked chicken strips 54 degrees F, cut tomatoes 45 degrees F, cooked beef slices 48 degrees F, and wrapped vegetables 46 degrees F.
- The following foods were found stored in the walk in cooler and inside the bottom shelf in the prep cooler without the date of preparation or the day by which the food shall be consumed: Yellow curry, cooked chicken, spicy red sauce, cooked beef in plastic bag, and cooked white noodles.
- Observed soiled empty containers stored on the top of cooked ready to eat food (cut tomatoes, cooked chicken, Thai sauce) on the covered prep cooler.
- Observed the following foods were not covered inside the prep cooler: cooked strip chicken, cooked beef slices, yellow curry and red curry.
- Observed food stored on the floor in the freezer, in the walk-in cooler and on the floor of the kitchen next to and behind the dry storage shelves (raw chicken, boxes of chicken, distilled cooking wine, dragon chef distilled vinegar).
- Observed dirty wiping clothes on the cooking stove, on the prep table, and on the top of the prep cooler.
- Observed utensils such as butter knives, scisors, and chef knives stored in a dry conatiner located next to ready to eat foods on the top serving line of the cold holding prep cooler.
- Employees were observed using multiple plastic bags to store foods in the freezer, the walk-in cooler and in the prep cooler.
- Observed the surfaces of the cutting boards on the top of the prep coolers are scratched on sight and touch.
- Observed there was no test strip at the 3 compartments sink to measure the chemical concentration at the 3-compartment sink.
- Observed the 3 compartments sink not having a sanitizing solution readily available.
- The exterior surfaces of the following Equipements were observed with accumulation of dust, dirt, food residue and debris: the walk-in cooler, the big rice cooker, the chest freezer and the ice machine.

- When questioned, staff indicated that the chicken kabob coming off of grill was fully cooked. Temperatures taken of chicken kabob varied from 120 to 211 degrees F.
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: mixed greens and spinach leaves on the front cold holding display unit at 45 and 47 degrees F (at 11:07am) and at 49 and 54 degrees F at 12:12pm; spicy and plain hummus on the front display unit at 53 and 47 degrees F, respectively at 11:07am; falafel and arugula greens in Avantco 3 door reach-in in bread station.
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Mixed greens and falafel in the Avantco 3 door reach-in refrigerator in the bread station at 45 degrees F.
- A spray bottle of cleaner was not properly labeled with the contents and was stored on shelving with a rack containing clean utensils.
- Turkish salad w/ cut tomatoes, chick pea salad and corn salad in the Avantco 3 door prep refrigerator across from the display unit were found to have temperatures of 48, 50 and 48 degrees F, respectively. All products were date marked and marking indicated that they had been prepared earlier this morning.
- The following refrigeration units were not holding temperatures of 41 degrees F and below: Avantco 3 door reach-in refrigerator in the bread station at 45 degrees F; the salad/greens front cold holding display unit indicated 38 degrees F at 12:12pm, but product never achieved 41 or below on unit.
- The integral stoppers for the basins of the 3 compartment sink were in disrepair and the basins could not hold water.
- Coats and a purse/backpack were observed to be stored on the shelving used for single service items near the walk-in cooler.

- Several of the employees that were working with food were observed not wearing hair restraints.

- Several sanitizer buckets were stored on the floor throughout the facility.
- Containers of clean utensils were stored in a manner that could contaminate the parts that come into contact with food, handles were not inverted.
- Throughout the facility there was single service containers being used for storage of clean equipment.
- There were several wood cooking utensils being used in the facility.
- The Autochlor dishwashing machine was not properly working and could not test the concentration of the sanitizer.
- In the restroom used by females there was no trash can with a lid for sanitary napkins.

- Upon arrival, the 3 compartment sink was not set up properly. When questioned, staff indicated that they only wash and rinse equipment.
- A boxes of avocados was stored on the floor near the 3 compartment sink.
- Employees making drinks out front were not wearing proper hair restraints. (Those in back had hair nets on.)
- The wet wiping cloths in the kitchen were not properly stored in the sanitizer bucket in between use.
- Sponges observed to be in use for cleaning at the 3 compartment sink.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (chlorine).

- No documents with the most up-to-date signs and symptoms related to food borne illness for establishment.
- No written procedure established or posted.
- All food products in walk-in-refrigeration unit observed contaminated by a great build-up of mold/mildew being propagated by the cooling unit. (Walls, ceilings and storage racks covered in contamination).
- Dishware covered in contaminate as well as chipped and broken.
- Water at kitchen hand sink measured @ 88.
- Both bathrooms missing 'self-closing door.'
- Storage area observed in disrepair. Gate is broken. Doors broken/removed.
- Walk-in observed covered in contaminate. Walls, ceiling, door and floor require cleaning/sanitizing. Cooling unit also observed covered and dispensing the mold/mildew and food contaminate.
