• Observed potentially hazardous food (cheese, tomatoes, mayo, hot dogs, bologna)cold held at greater than 41 degrees Fahrenheit in prep cooler.
• Observed potentially hazardous food (hamburgers, chili) not held at 135 degrees Fahrenheit or above. See temp below. CA: Informed PIC, food items were in the time parameter to be reheated to 165 degrees for 15 seconds, then placed for hot holding.
• Observed cooked potentially hazardous food (hamburgers) not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours, and then from 70 degrees to 41 degrees Fahrenheit within 4 hours. See temp below. CA: Informed PIC, burgers were discarded.
• Provided food storage chart, and Time as a Public Health Control form.

• Observed food debris buildup on microwave ceiling. CA: Microwave will be cleaned.
• Observed employee on cook line and in dish washing area with no hair restraint. CA: Discussed with PIC that all employees working with food must have a hair restraint.
• Observed wet wiping cloths not stored in sanitizing solution when not in use. CA: Discussed wet wiping cloth storage with PIC.

• Observed raw beef stored uncovered in reach-in cooler. CA: Educated PIC.
• Observed can opener not cleaned at a frequency necessary to maintain food-contact surfaces clean to sight and touch. CA: PIC will have employee clean can opener.
• Observed raw fish in prep sink sitting at 55°F and turkey at 51°F. Observed shredded cheese on ice bath at 55°F and sour cream at 72°F. CA: PIC placed items back into cooler to rapidly cool since they were out for less than 4 hours.
• A follow-up inspection will be held, but no date was provided.
