  • Observed potentially hazardous food (cheese, cucumbers, tomatoes) cold held at greater than 41 degrees Fahrenheit. CA: Food was held for more than four hours and was discarded. Facility will use ice bath in cooler until cooler will be repaired.
  • Observed scoops in sugar and salt not stored with handle facing up and out of food. CA: Discussed with PIC proper storage of scoops.

  • Observed no proof that employees are informed in a verifiable manner of their responsibility to report information about their health and activities as they relate to diseases that are transmissible through food. CA: Provided employee health policy.
  • Observed potentially hazardous food items not properly cold held at 41 °F or below, (whipped cream: 54 °F, cream cheese: 62 °F, milk: 51 °F). CA: Food items will be placed on TPHC from now on. Provided policy and explained process to PIC.
  • Observed incorrect inspection report displayed at front counter. CA: PIC will place new inspection report on counter.
  • Observed cups used as scoops in dry goods containers. CA: Informed PIC that approved scoops must be used, with handles sticking up above food.

  • Active managerial control measures are not satisfactory for establishment to be in compliance with code. No PIC was designated at the time of inspection and a certified food safety manager could not be verified for the facility. CA: Discussed with owner, a PIC was designated and it was stated that a proper certified food safety manager will be obtained.
  • Unable to verify the certified food safety manager for the facility. CA: Informed PIC that there must be a current CFSM for the facility and that the CFSM must be the person in charge or designate a PIC when not on the premises. The CFSM must supervise and insure proper handling of food and offer training to food service employees with regard to food safety at the facility.
  • Observed a container of ranch with mold-like growth. CA: Informed PIC, ranch was discarded.
  • Observed chlorine sanitizing solution in wiping cloth bucket and 3-comp sink exceeding the maximum concentration allowed. CA: Informed PIC, sanitizer solution was changed.
  • Observed containers of oil stored on the floor in the main kitchen and a box of eggs stored on the floor in the walk-in cooler. CA: Informed PIC that food items must be stored at least 6 inches off the floor, items were moved.
  • Observed no copy of the latest inspection report. CA: Informed PIC that a current copy of the inspection must be posted for public view. Inspection was then posted.
  • Observed dust accumulation on ceiling and wall above prep counter and 3-comp sink. CA: Pointed out to PIC that areas must be thoroughly cleaned.
  • Observed grease build-up behind cooking equipment. CA: Informed PIC that area must be thoroughly cleaned.
  • Remarks: Training was scheduled with facility; risk control plan will be provided.
  • This was a follow-up inspection to the restaurant's previous score of 65, or U.
