Make sure to stored food in a way that will allow items to be cooled to 41 degrees F within the required 6 hour time frame.
• Observed ready-to-eat, potentially hazardous food prepared on site that was frozen and then placed in the WIC without a new date marking. CA: Item was corrected on site. When items are removed from freezer to continue cold holding, a new date marking must be added to reflect the new discard date of the items.
• This is a follow-up inspection to the restaurant's previous score of 71, or C.

• Observed bed bugs on bedding and head boards in room 336. CA: Legal notice was issued due to failure by facility to take effective measures to eliminate the premises of beg bugs. Families must be removed from infested room as well as rooms in close proximity (above, below, to the left and right). Plan of correction was established and PIC was instructed to employ a licensed professional for treatment process. Follow-up scheduled for November 13th, 2018.
• Observed build up of green substance on shower heads in all inspected rooms except in rooms 335 and 236. CA: All lavatory fixtures which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists.
• Observed stained mattress covers and mattress in all rooms inspected: 206, 236, 235, 303, 309, 316, 328, 339, 337, 336, and 335. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin.

• Observed food employee touching ready-to-eat food with their bare hands and the establishment has no approved Alternative Operating Procedure. CA: Item was discarded onsite. Food employees shall not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment.
• Observed pan of quinoa on steam table at 100 degrees F. Reheating for hot holding shall be done rapidly and the time the food is between the temperatures of 41°F (5°C) and 165°F (74°C) and 41°F (5°C) and 135°F (57°C) for commercially processed food, may not exceed 2 hours. CA: Item was discarded.
• Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit (see "temperature observations"). CA: Items without time to cool were discarded onsite. Items with time to cool underwent rapid cooling procedures (placed on ice) and were placed in walk in units able to maintain proper temperatures. Education provided for cold holding issues observed.
• Observed improper test strips (QUAT strips) provided when using chemical (chlorine) sanitizer at three-compartment sink/warewashing machine. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. CA: Advised PIC to get proper test strips. Test strips were found onsite.
• A follow-up inspection will be held Nov. 15.
