Cook County Department of Public Health – Southwest Suburban Office

Location: 5th District Court Building, 10220 S. 76th Ave., Bridgeview
Hours: 8:30 a.m. to 4:30 p.m. M-F

Services provided: Immunizations (free)
What to bring: Parents must bring past immunization records or the school’s immunization requirements, a photo ID (of the parent) and proof of address.

Services provided: Physicals (with UA, $45, with UA and TTd, $55, sports physical, $45); immunizations ($20 for administration with Medicaid for patients under 18; cost of vaccine is covered under Medicaid besides administration fee)

Services: Student sports physicals, annual school physicals ($39; normally, $60) and immunizations.
