Medford Schools Open Online Kindergarten Registration

Here's how you can register for kindergarten online.

MEDFORD, MA — Medford Public Schools announced online kindergarten registration starting Tuesday, April 7.

See below for how to register and what's required, according to the Medford Public Schools:

1. Use the following link to input registration information. https://medford.schoolbrains.com/onlineapplication/enrollment.aspx. You will receive a confirmation email that your application has been received
2. Submit required documentation as listed below to: onlineenrolll@medford.k12.ma.us.
3. Submit health documents to: smccann@medford.k12.ma.us see below for health registration requirements.
Once your application has been received, it will be processed. Please take the time to scan/photograph the following documents to onlineenroll@medford.k12.ma.us. Notification of receipt and/or missing information will be sent to you by Maria Ibrahim, (781 393- 2132) at the Parent Information Center. Students cannot be enrolled without a Birth Certificate/Immunization Record and Proof of Residency.

Documents Required:
– Parent’s identification: license or passport
– Birth Certificate
– Health/Immunization Record
– Proof of Residency (two current utility bills, (i.e. water, gas, electric, mortgage
statement or lease) in the parent or legal guardian’s name
– English Language Learner Survey Form
– Early Childhood Survey

A school nurse will review your child’s health information for clearance into school in order to maintain protection from communicable diseases, identify your child’s health needs and promote health safety in school.

The following is a list of health requirements required by the Massachusetts Department of Public Health for entry of kindergarten students into public school. Medical documentation from your primary care physician needs to be emailed to Stephanie McCann, M. Ed, RN School Nurse smccann@medford.k12.ma.us for review.

Free immunizations and tuberculosis screening are available at the Medford Board of Health by appointment. Please call 781-393-2448.
– Physical Exam: completed within the last 12 months – Lead Screening with Results
– Vision Screening with stereopsis exam – Tuberculosis skin test or risk assessment with results

Immunizations:
– Hepatitis B: 3 doses (last dose after 6 months of age) –
– DTAP (Diptheria, Tetanus, Pertussis): 5 doses (last dose after age 4 years)
– Polio: 4 doses (last dose after age 4 years)
– HIB (H influenza bacteria): >3 doses
– MMR (Measles, Mumps, Rubella): 2 doses (first dose after age 12 months)
– Varicella (Chickenpox): 2 doses (first dose after age 12 months)
