Dayton Public Schools has received federal funding for technology and internet services for students through the Emergency Connectivity Fund. In order to ensure the district has correct household information, please complete the form linked below and return it to your student’s school. If you already completed this survey, you do not need to complete it again.

If you need to complete this form but do not have access to a printer, please contact your school for a printed copy.
