• Observed dishwasher with a chlorine concentration of ~0ppm. A chlorine solution shall have a minimum temperature based on the concentration and pH of the solution of 50-99ppm.
• Observed establishment operating without a current food service permit (1/1/18). Food service permit must be current and up to date. Advised PIC to contact Financial Services department.
• Observed heavy buildup and torn gaskets on reach in coolers in main kitchen and freezer in bar area. All physical facilities must be in good repair. C/A: Advised PIC to clean and/or replace gaskets on units.
• Observed heavy grease build up throughout facility's floors, walls and ceiling. Floors, walls, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. C/A: Advised PIC to clean facility.
• Observed missing light bulb under ventilation hood. Adequate lighting is needed in food prep, storage and service areas. C/A: Advised PIC to replace light bulb.

• Observed two cucumbers with mold-like growth in dry storage area. Food shall be safe, unadulterated, and honestly presented. COS: Cucumbers were discarded.
• Observed improper vertical separation of raw shrimp and raw sliced green peppers. Food shall be protected from cross contamination by separating raw ready-to-eat food during storage from other raw animal food. COS: Shrimp was moved to the bottom rack of RIC.
• Observed bowl of flour and container of cooking oil stored uncovered on rack in dry storage area. Food shall be stored covered to protect from contamination. COS: PIC covered bowl and container.
• Observed two small pests inside ice machine. Equipment food-contact surfaces and utensils shall be clean to sight and touch. CA: Pests were removed and advised PIC to burn existing ice.
• Observed TCS foods (chicken and lamb) cold held at greater than 41°F in RIC. See temperature observations. TCS food shall be maintained at 41°F or below at all times. CA: Containers were moved to another RIC to cool down.
• Observed cooked chicken sitting out on rack in main kitchen at 98°F. All TCS foods shall be maintained at 135°F or above all times. CA: Chicken was moved to oven to reheat.
• Observed heavy buildup on gaskets in reach in cooler in main kitchen. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
• Observed cell phone and keys located on prep top table in main kitchen. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. COS: Cell phone and keys were removed from prep table.
• Observed roach activity in main kitchen near RIC, RIF and inside ice machine. The presence of insects and other pests shall be controlled to minimize their presence on the premises. CA: Pest control reports were shown from October and November.
• A follow-up inspection will be held Dec. 14.
