• Observed several TCS foods labeled 12/10/18 held above 41 degrees in walk in coolers. See temperature observations. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. C/A: Advised PIC to cool foods in smaller batches and/or remove individual plastic wraps.
• Observed utensil handles touching food product in seasoning containers in dry storage area. During pauses in food preparation, dispensing utensils shall be stored in the food with their handles above the top of the food and the container. COS: PIC removed handles from food products.
• Observed leak in dishwashing area. A plumbing system shall be repaired according to law; P and maintained in good repair.
• A follow-up inspection will be held Dec. 18.

• Observed food stored in main kitchen reach-in cooler #3 failing to maintain temperatures of 41 degrees F or lower (see temp observations). Items without time to cool were discarded onsite. Items with time to cool were placed on ice. Inspector scheduled to conduct a follow-up inspection on Thursday, Dec. 13 to ensure that prep top cooler is in working order.

• Observed cooked chicken being left out at room temperature next to grill and observed at 61 degrees F. CA: Chicken was discarded onsite. Inspector informed PIC that food must either be cold held at 41 degrees F or below or hot held at 135 degrees F or higher. Food may not be left in the temperature danger zone, even if the food will eventually be reheated.
• Observed cooked fish being hot held at 100 degree F at front food service steam table. CA: Item was reheated to 167 degrees F and placed on steam table. All hot held items must maintain temperatures of 135 degrees F or higher.
• Observed raw, frozen beef being thawed at room temperature. CA: Thawing must take place under running water or in a refrigerator. Item was placed in a refrigerator to gradually thaw out.

• Observed failure to have paper towels at handwashing sink at front food service area as well as the main kitchen handwashing sink. CA: All handwashing sinks must be stocked with soap and paper towels. Both handwashing sinks were restocked during time of inspection.
• Observed food stored in main kitchen walk-in cooler #3 with mold-like growth. CA: Item was discarded onsite. Unsafe food must be discarded.
• Observed foods stored in salad bar area, grill prep area, and walk-in cooler # 3 failing to maintain temperatures of 41 degrees F or lower (see temp observations). CA: Items found out of temp were discarded onsite. After speaking with PIC, Inspector was informed that items are only held in salad bar area for 4 hours and all left overs are discarded onsite. Inspector advised that facility begin maintaining a time/temperature log to ensure items placed in salad bar area start off with adequate cold holding temperatures and are then discarded after time is up.
• Observed items stored in front food service area as well as items stored in main kitchen drawer warmer failing to maintain temperatures of 135 degrees Fahrenheit or higher (see temp observations). All items found out of temp were reheated to 165 degrees F or higher. All hot held items must be stored at temperatures of 135 degrees F or above.
• Observed failure to have test strips for quat sanitizer at three compartment sink. CA: Must obtain.
• A follow-up inspection will be held Dec. 26.
