Man Dies in Johnston Industrial Accident

Police and state and federal workplace safety officials are investigating.

JOHNSTON, RI—A 22-year-old man has died after a workplace accident at a Plainfield Pike recycling facility.

Police said Darwin Perez, 22, of Providence, was killed when he was using a belt-driver material sorting and crushing machine.

Police were dispatched to J.R. Vinagro on Plainfield Pike shortly after 8:30 a.m.

The state medical examiner, Johnston police and officials from the Occupational Safety and Health Administration are investigating.

In a statement, J.R. Vinagro said the entire company is devastated by the incident.

“During this time of great loss, our deepest condolences are with our team member, his family and friends. The safety of our team members is always our number one priority, and we are cooperating fully with local and federal agencies with the investigation of this tragedy,” the statement read.
